Come and See the Difference. With health care seemingly in a state of flux, there has been an ongoing search to find a more efficient and effective way to deliver health care services. One such way that has come to the forefront is called Patient-Centered Care. Essentially the patient becomes a partner in their health care and the care that is provided is respectful of their needs and values. The principles of Patient-Centered Care are as follows: Access to Care Continuity Involvement of family and friends Emotional Support Physical Comfort Information and Education Integration and Coordination of Provider Services Respect for Patient Preferences This is a good starting point and should be woven into the fabric of every patient relationship. However at Evergreen Cottages we think there is a higher calling…especially with the needs of dementia residents. We call it a Heart-centered approach. The difference is priorities.
