Dr. Krivak received his medical degree at Ohio State University College of Medicine and performed his residency at David Grant Medical Center, Travis Air Force Base in California. He completed his fellowship at the Walter Reed Army Medical Center in Washington, DC and at the National Cancer Institute, Rockville, Maryland.
