The call center staff will assist you and contact the physician on-call to address your concerns. We encourage you to sign up for the MyChart patient portal, which offers patients personalized and secure online access to portions of their medical records. Trinity Health IHA Medical Group, Primary Care - Rochester Hills is a designated Patient-Centered Medical Home (PCMH).
